Getting a DOT medical card – fast – at CareWell Urgent Care.
The Department of Transportation (DOT) physical and a DOT medical card are essential parts of medical documentation for any commercial driver. When drivers lack a DOT medical card, they can end up sidelined for days or weeks, taking trucks off the road and creating operational nightmares. That’s why CareWell Urgent Care makes getting a DOT physical fast and easy, allowing your drivers to get their DOT medical card as quickly as possible.
At CareWell Urgent Care, we offer 17 DOT physical locations in Eastern Massachusetts and Warwick, Rhode Island. Your drivers can get a same-day DOT medical card examination by walking into any clinic – or better yet, calling ahead or checking in online for an appointment to save even more time. Our clinicians are certified by Federal Motor Carrier Safety Administration (FMCSA) to provide DOT health card exams for drivers seeking their Commercial Driver’s License.
The cost for an exam for a DOT physical card is only $125, all fees included.
How the DOT medical card examination works.
Drivers seeking a DOT medical card physical at a CareWell Urgent Care clinic can help save time by completing the driver’s part of the Medical Examination Report before they arrive for their physical. Drivers should also bring:
- A list of their medications and dosages.
- Their glasses, if they need a prescription for vision.
- Their last Medical Examination Report card.
- A yearly clearance letter from a specialist, if they have a specific chronic condition.
During the appointment, drivers will undergo a thorough physical examination, as well as an eye examination, a hearing test and a urine test. Drivers with heart conditions may be required to take a heart test as well. The DOT medical card examination also includes a detailed review of a driver’s health history.

After the examination, our clinicians will complete the Medical Examination Report along with any supporting forms and documentation. Drivers passing the report can then be certified for a Massachusetts or Rhode Island State Commercial Driver’s License (CDL) or for a renewal license.
In addition to DOT medical card examinations, we provide a variety of occupational medicine services. CareWell Urgent Care clinics offer employee drug testing, Worker’s Comp exams, respiratory fitness tests, blood and urine labs, general physicals, EKGs and ER or urgent care for non-life-threatening medical needs.
