A DOT health card is an essential piece of medical documentation when driving commercial vehicles. To keep your drivers on the road, you often need to help them get a Department of Transportation (DOT) physical and a DOT health card as fast as possible. Scheduling a DOT physical with a regular doctor’s office can sometimes take days or weeks, which can create an operational nightmare if drivers are unable to work during that time.

How to prepare for a DOT health card examination.
The cost of the DOT health card exam is only $125, with all services included. Before your drivers arrive at our urgent care centers, they can fill in the driver’s portion of the Medical Examination Report to help save time at their visit. They should also bring a list of their medications along with the dosages, and their eyeglasses if they need a vision prescription. Drivers with a specific chronic condition should have a yearly clearance letter from a specialist, and all drivers applying for renewal should bring their last MER card.
During the DOT health card physical, drivers will have a thorough physical examination that will include:
- A DMV eye exam.
- A hearing test.
- A urine test.
- A heart test, for patients with any cardiovascular issues.
- A detailed review of health history, including any illnesses or injuries, vision problems, hearing issues, heart disorders, high blood pressure, muscular weakness or disease, digestion issues, diabetes, respiratory problems, and any issues associated with the lungs, kidneys, liver or nervous system.
Issuing the DOT health card.
For drivers that successfully pass the DOT physical card exam, our clinicians will complete all the information on the medical exam report and supporting forms and documentation. To obtain the Commercial Driver’s License DOT health card, all DOT physical requirements must be in place.
Successful completion of the DOT health card exam will result in certification for or renewal of Massachusetts and/or Rhode Island State Commercial Driver’s License (CDL) for truck drivers who are driving commercial trucks, and for school bus drivers.
